NIGHTMARE MODE: Handgun can now randomly "pop" heads off. Less ammo found for weapons other than the handgun. More damage done to player, and more stronger enemies. Enemies move faster. Zombies are only killed by blowing their heads off. Zombies can be killed without decapitating their heads but they will continue to get up after being knocked down 3 or more times and can take full clips of ammo before dropping.
